This is how I do it. This method might not work on EVERY image (like greyscale) but it should on most.

Start by picking a picture. It can be any size, even wallpaper size. Here's what I picked: http://img148.imageshack.us/img148/853/403068fr7.jpg

1. Resize the image. Go to Image > Image Size. I changed mine to 150x186.

2. Use the Rectangular Marquee Tool, style: Fixed Size, 100x100 and click on the part of the image you want for the icon. Once it's selected, hit Ctrl + C.

3. Make a new canvas and paste your image. File > New > OK, Ctrl + V.

4. Duplicate the base (right-click, Duplicate Layer) and hit Ctrl + U. Move the Saturation all the way up to +100. Click OK. Then set the layer to Soft Light at 50%. Merge the layers (right-click, Merge Visible)

5. Duplicate it again, hit Ctrl + U, but this time check Colorize. Set it to Hue: 180, Sat: 100, Light: -25, then click OK. Set it to Soft Light at 50%, and merge the layers again.

4. Now sharpen the image. Filter > Sharpen > Sharpen.

5. Duplicate the base and set to Screen at 25%.

6. Duplicate it again, and change to Soft Light at 35%.

7. Create a new layer and fill it with the colour #F5D4B7, and set it on Soft Light at 50%.

8. Create another layer and fill it with the colour #0A0C2C, and set it on Exclusion at 100%.

9. Right-click, Merge Visible. Add any other effects that you want.
